KEY COMPONENTS OF THE PROGRAM ARE THE COMPREHENSIVE HEALTH CARE CENTERS AND THE INTERDISCIPLINARY TEAM. THE COMPREHENSIVE HEALTH CARE CENTERS SERVE AS THE SITES WHERE THE ELDERLY PERSON RECEIVES MOST SERVICES IN ONE SETTING. THE INTERDISCIPLINARY TEAM IS COMPRISED OF HEALTH CARE PROFESSIONALS AND ALLIED HEALTH STAFF WHO EVALUATE, DEVELOP A CARE PLAN, AND DIRECTLY PROVIDE ALL NECESSARY SERVICES.
